You may like to include a link to the original writeup here:

so people can see the original rationale for that rather rough mockup
and also since it's a bit disturbing to see the stuff I wrote copied
and re-worded with "I propose" preceding it.
The main difference between your image and mine is that mine resembles
the Mac OS X 10.3 file selector and yours, the Windows one. If we're
going to be making something similar to an existing one, it would be
best to break down and analyse the difference between existing file
selectors on their own merits in order to decide. I personally find the
10.3 file selector less cluttered, and easy to to see buttons at a
glance through positioning. The placement of the 'action' and cancel
buttons side by side, rather than above and below, makes it easier to
hit one of them quickly and accurately, and the consistency of the
cancel button always on the left makes it even faster, since you don't
even have to read the text - you know what button is which, by the
position and can hit it at a glance. The Windows one isn't quite so
fast because you have to be very careful to get the Y-co-ordinates of
your click right in order to hit the correct button. Like in 10.3, I
also did neglect a field to type in the filename for the open dialog,
as it's only necessary when saving.
One more thing, all the icons and graphics are just placeholders of
course, but it's important keep the semiotic meanings of icons intact
by using the blender file icon to represent .blend files, *not* the
Blender application icon.
