[Bf-cycles] Volumetric patch update (now with new shiny bidirectional integrator)

Few cleanups, zero actual changes, trying to refactor kernel_mlt.h to
respect blender coding style (passing through indent --linux-style
--line-length 132), also first try of long promised removing of very old
stupid attempts leftovers that make code very unreadable.

More tests done, with hard scenes that have areas that equire long path
length, and must admit MIS work well.
