You may want to consider using the script that ships with Nvidia's drivers as a
base.  The name escapes me at the moment, but it's the script that Nvidia asks
everyone to run before they submit a bug report.  It would need a bit of
tweaking, but it would gather a lot of the same information we'd want to see as
well.

If you'd like help one the script for linux as well, I might be able to provide
some help.  Just let me know what you need if you're interested in assistance
in that.
