These are the events, reported by xev, when pressing ALT-CTRL-RMB:

So it seems, that my Window Manager does not swallow those events
(?)...

Is it possible, that different versions of X code keyboard/mouse
events in different ways?

I had such a case with Microemacs (Jasspa) when using right ALT (ALTGR
on german keyboards) on XFree 4.3.x and Xfree 4.4.0 .
