Hi Stefano,
I have used, with some success, this chain of products (Not free!!!):

        1) GemDoc for DocBook to RTF
        2) Microsoft Word for some arrangement
        3) Adobe Acrobat (Plugin for Word) for export into pdf

The result is an hi quality pdf (Bookmarks+links)
